AIR PT FORMS

The following forms are made available by Air Physical Therapy.  Each form may be printed out, completed, and submitted to Air Physical Therapy.  The forms are a simple way for Air Physical Therapy to learn more about a patient before the physical therapy process begins.

• Patient Registration Form
  The Patient Registration Form is a two-page form allowing the patient to fill out his or her personal contact injury information, insurance information, and authorization towards Air Physical Therapy.

• Patient Health Questionnaire Form
  The Patient Health Questionnaire Form is a two-page form for patients to fill out with health information.  Patients may fill out current symptoms and medications as well as any past medical history.
